How does HIFU work?

Before facial or body HIFU treatment, a conductive gel is applied and the appropriate handpiece is selected to emit focused ultrasound waves, which work at depth without damaging the surrounding skin. Depending on the handpiece, the waves penetrate between 1.5 and 9 mm, reaching the dermis, connective tissue and SMAS, which are responsible for the lifting effect on both the face and body.

The heat generated contracts and stimulates collagen, promoting its renewal and producing a visible firming of the skin.

Number of HIFU sessions

The number of HIFU sessions is determined according to the condition of the patient’s skin and their expectations. Normally, 1 session is enough to achieve visible results, but when two sessions are required, an interval of 3–6 months is recommended.

How does HIFU differ from a surgical facelift?

Frequently asked questions about HIFU

The results achieved with HIFU are maintained, although ageing is an ongoing process. When HIFU is performed as a preventive treatment, several years may pass before it needs to be repeated. When laxity is already established and the results are to be maintained, a maintenance schedule of one session per year is generally recommended, although every case is different.

Facial HIFU may be combined with neuromodulators in the upper third of the face, the masseter muscles and the platysma when necessary, or with radiofrequency, lifting threads and other treatments.

For body HIFU treatments, the presence of fat and/or muscle laxity must always be assessed in order to combine HIFU with other treatments. For laxity, body HIFU can be combined with devices that produce supramaximal muscle contractions and improve muscle tone, as well as collagen stimulators.

During HIFU treatment, the patient may experience mild discomfort consisting of small, brief points of heat that last only while the ultrasound energy is being emitted. This sensation is felt at depth because HIFU acts on the deeper layers of the skin, where remodelling the structures improves laxity and tightens the skin from within.

The level of discomfort depends on the settings used on the HIFU device, including energy, handpiece depth and pitch—the distance between ultrasound emission points—as well as the area being treated and the patient’s sensitivity.

Treatment around the eyes and over bony areas tends to be less comfortable. The neck is generally the best-tolerated area. HIFU can be started at a lower energy level and increased progressively. Thicker or denser skin also tends to tolerate HIFU treatment better.

The treatment HIFU can be performed at any time of year, as it does not affect the superficial layers of the skin or increase sensitivity to the sun.

However, there are some seasonal recommendations:

  • In summer, facial HIFU can be performed without any problem, provided that the following precaution is followed: use SPF 50 sunscreen (sun protection should always be used, even when no treatments are being performed).
  • After intense sun exposure when the skin has suffered sun damage, it is advisable to wait a few days until the skin has fully recovered.

In summary: the HIFU treatment is safe throughout the year, provided that it is performed on healthy skin and basic sun-protection precautions are followed.

Following HIFU treatment, whether facial or body HIFU, the possible side effects are:

  • Mild redness that may last for up to one hour.
  • Mild tenderness to the touch or when moving the treated area for 1–2 weeks.
  • Mild bruising caused by the rupture of blood vessels in the soft tissue. It usually resolves within two days to two weeks after treatment. Patients receiving anticoagulant therapy should therefore not be treated, and the transducer should not be applied directly over a visible blood vessel.

The contraindications for facial and body HIFU treatment are:

  • Open wounds in the area to be treated.
  • Metal stents or implants in the face or neck; dental implants are not a contraindication.
  • Electrical implants.
  • Within two weeks after treatment with neuromodulators.
  • Pregnant or breastfeeding women.
  • Anticoagulant therapy.
