Different types of wrinkles, different treatments

Expression lines develop as a result of repeated facial muscle movement. They commonly appear on the forehead, between the eyebrows, around the eyes (crow’s feet) or around the mouth (perioral lines). They are treated with neuromodulators.

By contrast, static or gravitational wrinkles develop over time as the skin ages, either naturally or as a result of sun exposure. The most common are found in the nasolabial folds, on either side of the mouth. In these cases, the recommended treatment is hyaluronic acid.

How does neuromodulator treatment work?

Neuromodulator injections contain substances that block muscle movement and are administered directly to the areas being treated, such as expression lines around the eyes, on the forehead and between the eyebrows.

Their effect relaxes muscle movement, reducing more pronounced wrinkles, eliminating mild ones and even helping to prevent wrinkles.

Number of sessions and when results appear

Results may begin to appear 3–5 days after treatment, reaching their maximum at around 14 days depending on the case, and usually last approximately 3–6 months.

Results depend on the severity of the lines and the body’s response to the product, as it is a medicine. Reduced or absent muscle movement can be observed, so wrinkles are softened or eliminated depending on their severity.

Patients can resume their normal activities after receiving the injections.

A review is recommended after 2 weeks.

Aftercare

After treatment with neuromodulators, it is recommended to:

  • First 4–6 hours: do not lie down or bend forwards, do not massage the treated area, avoid strenuous exercise and avoid exposure to intense heat.
  • During the first 24 hours: avoid sleeping face down or on your side, and do not have facial treatments or deep-cleansing treatments.

Frequently asked questions about facial neuromodulators

The effect of injections of substances that block muscle movement and correct wrinkles caused by facial muscle activity takes place at the neuromuscular junction. In this transition area between the peripheral nerve and the muscle, acetylcholine is released, a neurotransmitter required for muscle contraction. Neuromodulator injections act locally by blocking the release of acetylcholine, resulting in temporary muscle paralysis.

The results of neuromodulators for wrinkle treatment usually last between 3 and 6 months, depending on factors such as each patient’s individual response, the area treated and the type of product used.

After this time, the effect gradually wears off and further sessions are recommended to maintain smooth, natural-looking skin.

Neuromodulator injections are not usually painful. They are administered using very fine needles and, in many cases, a topical anaesthetic is used for greater comfort.
Most patients describe the sensation as a small pinprick or mild discomfort, which is very tolerable and short-lived.

Allergic reactions to neuromodulators are very uncommon. These treatments have been widely tested and have a strong safety profile. In most cases, patients do not experience any allergy or significant adverse reaction.

For this reason, it is always important for the procedure to be performed by a medical specialist, who can assess your medical history and ensure safe treatment. It is important for the doctor to take a medical history to assess the presence of autoimmune or muscular conditions.

The use of aesthetic neuromodulators for wrinkles is a safe treatment, but some mild and temporary side effects, may occur, such as:

  • Redness or swelling in the treated area.
  • Small bruises or local tenderness.

  • Rarely, slight asymmetry or temporary eyelid drooping, which resolves over time.

In general, neuromodulators do not have long-term effects when administered correctly and at appropriate doses.

However, some physiological and clinical aspects should be taken into account, such as the fact that there is no scientific evidence of cumulative toxicity (the toxin is not stored in the body; its action is local and it degrades over time). In addition, studies with more than 20 years of follow-up have been carried out in patients receiving therapeutic rather than aesthetic treatment for conditions such as dystonia or spasticity, with no relevant systemic effects found.
